Internship Paperwork


In order to receive credit for the internship, a student must make have the following paperwork submitted:

Internship Agreement

This should be completed at the start of the internship. It includes basic data on the internship and completed by both the student and employer showing they agree to the terms of the internship.

Mid-term Report

This is completed by the student mid-way through the internship or after 75 hours. The goal of the mid-term report is to check the status of the internship and determine if any changes need to be made to the internship.

Student's Final Report

This essay is completed by the student at the end of the internship to allow the student to reflect on the internship experience and what was learned. It also provides basic information to the department for assessment and portions may be published to the web for future students.

Supervisor's Final Report

This survey is completed by the intern's supervisor at the end of the internship. Data from these reports are used by the CS/CIS department to evaluate our program and identify areas in the curriculum that need improvement.
